With the support of a dedicated Wellbeing Specialist who truly understands you, you’ll have someone to turn to, whether planning for the future, enhancing daily life, or navigating a crisis.

Our recommendation: If you'd like a trusted specialist to get in touch with you every month then go for the Flourish Plan, if you're comfortable with a video call get started with Foundations, and if you want to see us in person with an occasional check-in then chose the Connected Plan.

What to Expect
For those living at home

Our Wellbeing Specialist will get to know you and your family. They will conduct an assessment looking at physical, emotional, social and cognitive wellbeing. They will also discuss lifestyle, activities and general mood with you.

You'll receive a personalised report and a plan of action citing ways in which the family or care team can improve and enhance wellbeing. This could be simple things like bringing some plants indoors for someone that was a keen gardener or getting more support around the house.

You should start to notice a positive change in things like engagement, fulfilment, social connection and outlook.

Regular check-in calls with our Specialist will monitor the situation and they will advise on any necessary changes. These reports will be easily accessible, so that everyone is kept up to date.

Our annual assessment ensures that the best possible quality of life is maintained.


For those in a care home

One of our Wellbeing Specialists will get to know you and work alongside your care provider to conduct an assessment looking at all aspects of wellbeing including physical, emotional, social and cognitive wellbeing. They will also discuss lifestyle, activities and general mood.

Our Specialist will provide a personalised report and action plan, supporting the care home team to enhance and improve wellbeing.

Regular check-in calls with our Specialist will monitor the situation and they can advise on any necessary changes. These reports will be easily accessible, so that the care home and family is kept up to date.

Too many older people end up in hospital after an illness or a fall, and never leave, or if they do it often compromises their independence and autonomy. When you have our wellbeing support in place then you know we are on hand to act quickly and advocate for you. We can provide additional consultancy* at a time of crisis to ensure wellbeing and quality of life is the primary consideration in decisions about your future.
*Consultancy will incur an additional fee charged at an hourly rate.
